Onychocryptosis - Ingrown toenail

Onicocriptosi - Cause

It is a podiatric problem characterized by the penetration of the nail plate in the periungual edge.

This condition determines a lesion in situ that clinically manifests itself with pain on pressure (usually on contact with footwear), an evident inflammation, hyperaemic skin and, in more complicated cases, the presence of blood and granulocytic exudate (pus) up to the formation of a granuloma. All this is due to the fact that the penetration of the foil inside the perionychium (region surrounding the nail) is registered by our body as a foreign body, consequently activating all the defense mechanisms of our body.

CAUSES
The conditions that determine the onset of onychocryptosis see in 50% of cases an incorrect cut of the nail plate, in 20% of cases an alteration of the nail matrix, in 15% a direct or continuous trauma with the shoe, in 15% biomechanical alterations of the big toe. In addition, conditions of hyperhidrosis (excessive sweating), skin dryness or onychophosis (hyperkeratotic thickening in the periungual edge), but especially neglect and carelessness of the problem, can predispose to rather important clinical pictures.

Onychocryptosis - Treatment

The treatment of onychocryptosis involves the excision and subsequent removal of the nail portion cause of the lesion, without the use of anesthetics. Afterwards, specific medications will be performed by the patient and assisted by close controls by the professional podiatrist to achieve complete healing and therefore the restitutio ad integrum of the injured tissue.

If the nail plate is repeatedly predisposed to onychocryptosis, a nail re-education treatment (orthonixia) is indicated. Through this method, through the application of nitinol wires, resin caps, etc., it is possible to determine a small but sufficient traction on the nail to prevent it from damaging the peri-ungual edge again.
